St. Paul’s PACE provides comprehensive healthcare and support services for seniors 55 and older who have chronic medical conditions and need assistance to live safely at home. Through personalized medical care, medication management, physical therapy, home care, social work support, and transportation, St. Paul’s PACE helps seniors maintain their independence while receiving the care they need in a safe and familiar environment.
